How Accurate Is The Medical Science At The Heart Of Reachers Season 4 Premiere? – TVLine

The following post contains spoilers for “Reacher” Season 4.

The opening episode of “Reacher” Season 4 introduces Jack Reacher (Alan Ritchson) to the mysterious new characterLila Hoth (Agnez Mo). Her motivations are sympathetic: She has leukemia (a type of cancer affecting the body’s blood-forming tissues)and she’s having trouble finding someone for a bone marrow transplant. Lila explains to Reacher that, due to her mixed-race heritage, it’s difficult to find someone who’s a viable transplant match. That’s why she’s hatched a plan to blackmail presidential candidateJohn Sampson (Marc Blucas) into donating his bone marrow. Why him? Lila has found proof that she is Sampson’s illegitimate child, and she hopes to weaponize that information against him.

Does any of this hold up to medical scrutiny? “Reacher” has been a big hit (Prime Video renewed it for a fifth season ahead of the Season 4 premiere), but it’s not known for its strict adherence to realism. That’s why it’s refreshing to know that, according to the American Society for Clinical Laboratory Science, at least one part of Lila’s story checks out: It is indeed very common for people of mixed ethnicity to struggle to find life-saving donors.

“Their potential matches are so limited that they may have to seek individuals living outside of the United States just to find a match,” explains medical technologist Ogechi Ohaeto. “This is why it is very important to have people sign up to be bone marrow donors, especially those who are of ethnically diverse backgrounds.”

There is one major flaw in Lila’s plan

While the premise of Lila being this desperate to find a bone marrow donor is definitely believable given her mixed-race background, her strategy is somewhat flawed because Sampson would only actually be a 50% match. That’s because, per the Columbia University Herbert Irving Comprehensive Cancer Center, bone marrow matches are based on your human leukocyte antigen (HLA) typing. Whereas hereditary traits like eye color or blood type are often a perfect match to one parent, a person’s HLA is a mix of both parents’ DNA.

However, this doesn’t necessarily mean Lila’s plan would be doomed from the start. As the Herbert Irving website explains, “Half-matches, or haploidentical transplants, are increasingly common and effective thanks to advances in transplant medicine.” This means that, while Sampson might not be an ideal donor, there’s still a chance that his bone marrow might be able to save Lila. Of course, this is all assuming that Lila’s being fully honest with Reacher about her motivations. Based on everything we know about “Reacher” Season 4 so far, that might not be a safe bet.
